Midwifery Nursing | High-Risk Pregnancy and Obstetric Emergencies | Postnatal Mother Assessment Tool |
High-Risk Pregnancy and Obstetric Emergencies :
Preterm Labor, Preeclampsia, and Eclampsia :
Preterm Labor is the onset of labor before 37
weeks of gestation, leading to premature birth.
Causes:
- Multiple
pregnancies
- Infections
- Uterine
abnormalities
Management:
- Tocolytics
(e.g., nifedipine) to delay labor
- Corticosteroids
to enhance fetal lung maturity
- Bed
rest and hydration
- Preeclampsia:
High blood pressure (>140/90 mmHg) and proteinuria after 20 weeks
gestation.
- Eclampsia:
Severe preeclampsia with seizures.
Management:
- Magnesium
sulfate for seizure prevention
- Antihypertensives
- Emergency
delivery if severe
Gestational Diabetes and Hypertension
- Gestational
Diabetes Mellitus (GDM): High blood sugar during pregnancy, increasing
risk of fetal macrosomia.
- Gestational
Hypertension: Blood pressure elevation without proteinuria
after 20 weeks.
Management:
- Dietary
modifications
- Insulin
therapy if required
- Regular
fetal monitoring
Obstetric Hemorrhage and Cesarean Section Care
Obstetric Hemorrhage
- Postpartum
Hemorrhage (PPH): Blood loss >500mL (vaginal) or >1000mL
(C-section).
- Causes:
Uterine atony, retained placenta, trauma.
Management:
- Uterine
massage
- Oxytocin
administration
- Blood
transfusion if needed
Cesarean Section Care
- Preoperative:
Informed consent, IV line establishment
- Postoperative:
- Monitor
for bleeding, infections
- Pain
management
- Early
ambulation
Nursing Care Plan: Preeclampsia
|
Nursing Diagnosis |
Risk for complications related to high blood pressure and
proteinuria |
|
Assessment |
High BP, proteinuria, headache, visual disturbances |
|
Nursing Interventions |
- Monitor BP, urine output - Administer antihypertensives
- Educate on fetal movement tracking - Prepare for possible early delivery |
|
Expected Outcome |
BP stabilized, no seizures, healthy fetal outcome |
High-risk pregnancies require close monitoring and timely
interventions to prevent maternal and neonatal complications. Nurses play a
vital role in assessing, managing, and educating mothers to ensure safe
pregnancies and deliveries.
